Planejamento digital e plástica oclusal na reabilitação oral: relato de caso

Reabilitações orais extensas frequentemente requerem a modificação da Dimensão Vertical de Oclusão (DVO) para restabelecer função, estética e estabilidade oclusal, especialmente em pacientes com desgaste dentário e guias de desoclusão inadequadas. Este relato apresenta uma reabilitação oral total adesiva com aumento planejado da DVO, por meio de abordagem minimamente invasiva baseada em planejamento digital e materiais resinosos, denominada Plástica Oclusal. Paciente masculino, 52 anos, apresentava queixa estética e fraturas recorrentes nos dentes anteriores, associadas à ausência de guias oclusais e contatos excessivos anteriores. O planejamento incluiu registro da relação cêntrica com JIG, escaneamento intraoral, montagem em articulador digital e enceramento diagnóstico em software de código aberto (Autodesk Meshmixer®). O aumento da DVO foi estabelecido em 1,7 mm na região posterior, possibilitando a reanatomização anterior e restabelecimento das guias de desoclusão. Após fase provisória com restaurações impressas em 3D para validação funcional e estética, realizou-se a reabilitação definitiva com facetas diretas em resina composta e overlays table-top fresados por CAD/CAM. Observou-se estabilidade oclusal, conforto muscular, melhora estética e satisfação do paciente, evidenciando abordagem previsível e conservadora.
