ANTIBIÓTICOS E ANTI-INFLAMATÓRIOS NA ODONTOLOGIA: USO INDEVIDO, RISCOS E A URGENTE NECESSIDADE DE CRITÉRIOS CLÍNICOS

O uso de antibióticos e anti-inflamatórios na Odontologia é prática comum, mas frequentemente realizado de forma inadequada, sem respaldo em critérios clínicos claros. Esta revisão sistemática analisou publicações entre 2019 e 2025, incluindo diretrizes, revisões sistemáticas, ensaios clínicos e estudos observacionais. Evidências demonstram que antibióticos raramente são indicados em quadros de dor pulpar e periapical sem sinais sistêmicos, e que a profilaxia deve ser restrita a situações específicas, como pacientes de alto risco para endocardite infecciosa. O uso indiscriminado de clindamicina foi associado a maior risco de infecção por Clostridioides difficile, reforçando a necessidade de alternativas mais seguras. Em cirurgias odontológicas, como exodontia de terceiros molares e instalação de implantes, o benefício da profilaxia antibiótica é modesto ou incerto, e regimes prolongados não são recomendados. Para analgesia, AINEs isolados ou combinados com paracetamol mostraram-se superiores aos opioides, com menor perfil de risco. Os resultados evidenciam a urgência de programas de stewardship voltados à Odontologia, com o objetivo de racionalizar prescrições, reduzir riscos e alinhar a prática clínica às melhores evidências disponíveis.
