EVIDÊNCIAS CIENTÍFICAS SOBRE O USO DO L-PRF NA CICATRIZAÇÃO DE ÚLCERAS CUTÂNEAS: UMA REVISÃO SISTEMÁTICA

Resumo

A fibrina rica em leucócitos e plaquetas (L-PRF) tem sido proposta como biomaterial autólogo capaz de favorecer a cicatrização de úlceras cutâneas crônicas. Ao utilizar componentes do próprio sangue para modular inflamação, angiogênese e reparo, essa estratégia aproxima o cuidado clínico de uma abordagem biológica, acessível e personalizada. Esta revisão sistemática analisou estudos publicados nas bases PubMed/MEDLINE, Embase, Cochrane Library, SciELO e LILACS até 18 de outubro de 2025. Foram incluídos ensaios clínicos randomizados, estudos controlados não randomizados, coortes e revisões sistemáticas sobre aplicações extraorais de L-PRF em adultos com úlceras crônicas. Os principais desfechos avaliados foram cicatrização completa, tempo de fechamento, redução da área lesional, dor e eventos adversos. Foram incluídos 14 estudos (7 ensaios clínicos randomizados, 4 coortes e 3 revisões). Em úlceras venosas e diabéticas, observou-se tendência à aceleração da granulação, redução do tempo de cicatrização e alívio da dor, com perfil de segurança favorável. Em úlceras neuropáticas e infecciosas, os efeitos foram mais discretos. A qualidade global da evidência variou de moderada a baixa, limitada por amostras pequenas e falta de padronização. Conclui-se que o L-PRF se apresenta como adjuvante útil no manejo de úlceras cutâneas crônicas, sobretudo quando integrado a um cuidado abrangente que inclua controle clínico, desbridamento e manejo de infecção. Os resultados são promissores, mas requerem ensaios multicêntricos padronizados para consolidar a efetividade e definir seu papel na prática clínica.
