IMPACTOS INVISÍVEIS: DIAGNÓSTICO PRECOCE DA ENCEFALOPATIA TRAUMÁTICA CRÔNICA EM LUTADORES DE ARTES MARCIAIS

Introdução. Esportes de combate aumentam o risco de traumas cerebrais, associados desde 1928 à Encefalopatia Traumática Crônica (ETC). Com a expansão dessas modalidades, investigar a ETC em lutadores é essencial para reduzir riscos neurológicos e promover a segurança. Justificativa. A exposição midiática desses esportes reforça a relevância da pesquisa alertando sobre riscos, abordando a complexidade diagnóstica da ETC, orientando condutas e contribuindo no aperfeiçoamento da literatura. Objetivos. Analisar os métodos de diagnóstico precoce e as manifestações iniciais da ETC em lutadores, identificando as manifestações clínicas e exames complementares e propondo um fluxograma para diagnóstico ou suspeita clínica. Metodologia. Consiste em uma revisão sistemática embasada nas diretrizes PRISMA, utilizando a plataforma PubMed para buscar artigos publicados nos últimos 10 anos. A amostra inclui lutadores profissionais de artes marciais acima de 18 anos, e examina características clínicas e exames complementares, como alterações morfofisiológicas, cognitivas, motoras e comportamentais. Resultados e Discussão. Foram identificados exames de Ressonância Magnética (RM), Tomografia por Emissão de Pósitrons (PET) e Imagem de tensor de difusão (DTI) como principais para diagnóstico, apresentando maior relevância na detecção de alterações morfológicas. Exames neuropsicológicos e biomarcadores, como NfL e GFAP, são úteis para monitorar sintomas cognitivos e comportamentais. Conclusão. O diagnóstico precoce da ETC permanece desafiador. No entanto, uma abordagem multimodal combinando neuroimagem, biomarcadores e testes cognitivos oferece novos caminhos na identificação precoce da condição. Estudos longitudinais e o desenvolvimento de métodos diagnósticos específicos são necessários para prevenir danos irreversíveis e melhorar o manejo da ETC
